Community Building Together (CBT)

For more than twenty years, BData founder Bart Phillips has worked at the intersection of data science, healthcare innovation, and community development. Alongside building BData’s clinical analytics platform, Bart founded Community Building Together (CBT), an organization dedicated to strengthening communities and addressing the social conditions that shape long-term health outcomes.

Today, that commitment continues through Bart’s leadership as Board Chair of the Minnesota Community Health Worker Alliance (MNCHWA), a nationally respected organization advancing the Community Health Worker profession. Through these partnerships, BData is exploring new ways to connect advanced clinical data with community-based care — ensuring that innovation in healthcare reaches beyond the hospital and into the neighborhoods where health truly begins.

What is a Community Health Worker?

A Community Health Worker (CHW) is a trusted member of the community who helps bridge the gap between healthcare systems and the people they serve.

CHWs support individuals and families by helping them navigate healthcare services, access community resources, and overcome barriers that affect health — including housing, food security, transportation, language, and health literacy. Because CHWs often share lived experiences with the communities they serve, they are uniquely positioned to build trust, improve care coordination, and help patients follow through on treatment and recovery plans.

Across the United States, CHWs are increasingly recognized as an essential part of the healthcare workforce, improving outcomes while reducing unnecessary hospital visits and healthcare costs.
